Drop curb not flat to road - danger to cycling

The drop curb which connects the road to cycle lane is not flat. On Monday my partner was forced from the cycle lane onto the road 30m east along West Way by a car pulling across the bike lane. She then attempted to re-join the cycle lane but hit this drop curb and came off her bike, causing serious injuries. Please flatten the connection between road and cycle lane to enable safe transition from road to cycle lane. Whilst you're at it, several other of the drop curbs connecting cycle lane to road along Botley Road are of the same quality - not flat and therefore dangerous to cyclists. I nearly came off my bike further down Botley Road a week ago doing exactly the same transition between road and cycle lane but I should log this separately. I appreciate this bike lane is fairly new but it hopefully it will be easy to push down the concrete curbs and make it flat and safe for cyclists. Or even remove the concrete curb entirely and have completely flat tarmac to connect the road and cycle lane.

Sorry - a quick update, I just did some more research and it seems the drop curb connecting road to cycle lane should be 3-6mm high MAXIMUM. This is certainly not. Please make this safe.
