Failed manhole cover

Failed manhole on verge area just off road used for parking.

This was fixed about a month ago but has failed again. Most of the damage occurred when two workmen opened it, had a look down it, dropped the lid, re-opened it and then shut it again cracking all of the concrete holding the cover in place.
