Extremely loud manhole cover loose

Manhole cover is extremely loose, it has been attempted to be ‘covered’ up by tarmac, however the problem is now even worse. This has been ongoing for 2+ years. It’s extremely loud whenever a car drives over it. Unable to have our windows open because it is so loud and now affecting our sleep and mental health.
