Uneven, damaged footpath

I am writing to report an issue with the public footpath outside my home. The footpath is uneven and sinking and the manhole covers are uneven and a trip hazard. My Husband is registered disabled and the footpath directly outside our home is a trip hazard and in need of urgent repair .

The footpath has got worse over the last year and without maintenance is only a matter of time until it becomes a major hazard .

Please would you be kind enough to urgently look into this?
