Cracked and uneven pavement outside my drive. The large loose concrete slab lifts up when I enter my drive

Uneven path with cracked concrete. Tripping hazard. Loose concrete slabs raise and tip up when entering my driveway. Worried about damage to my vehicle being caused when the concrete tilts. Path has never been maintained to my knowledge in all the years I have lived here. Has gotten worse the last few years.
