Manhole cover on grass

This manhole cover was reported a very long time ago and the utility company placed a cone on it followed by a plastic barrier. There is a hole in the cover that has gotten worse and now the cover itself has started to shift off and there is a gap around the edge. This is quite dangerous as the plastic barrier no longer stands up. This should be urgently fixed.
