Collapsed and missing drain cover

Sunken rainwater drain cover has collapsed into hole. Now deep and very hazardous as wheels of smaller vehicles and bicycles will drop into it. Very narrow section of busy road, so vehicles often forced to drive over the affected drain, needs fixing urgently before major accident.

Photo attached. This has been reported twice over the last few months and in each in each case a temporary repair of tarmac around the drain has been done. The drain needs rebuilding properly. We' have placed a traffic cone over the drain as it is at a stage where is could take down a cyclist or damage a car tyre.
