Growing pothole and subsidence from old mend

Pothole has been consistently growing since the start of the year. It is now affecting cyclists to the extent that I have put a bollard ahead of it to prevent further issues. It's now large enough to damage car tyres. But that's not all. The section of road around it has mended before and is no subsiding, meaning that there are further holes developing.
