Utility cover plate has gaping hole underneath

A crack appeared 2 months ago at the edge of a utility cover on the pavement near the start of nuneham square. The hole is expanding and you can see through to underneath where a sink hole seems to be forming. It is only a matter of time before this problem gets a lot bigger- potentially with someone falling in when they stand on the cover
