Damaged access chamber causing a trip hazard and deep hole in footpath

The metal cover to an access chamber is damaged, loose and often dislodged creating a deep hole on the footpath on Buckingham Road. It is next to an illuminated roundabout signpost. The chamber needs repairing/replacing.
