Deteriorating and broken pavement surface, creating dangerous and uneven trip hazard

The tarmac surface of the pavement into areas in close proximity to each other, have broken up and completely disappeared, resulting in an uneven and dangerous trip hazard. A 71-year-old resident of the village fell as a result of this on 9th May, receiving lacerations to his face Please can a proper repair be done as soon as possible to avoid similar occurrences.
