Series of pot holes & broken up road for about ten metres

I was forced into this pot hole by a HGV overtaking me and I came off my bike into the ditch! The entire 5 metre section of road going out of the village is broken up & a serious hazard as cars are swerving onto the other size of the road on a blind bend. I have reported this issue a year ago, it needs properly resurfacing. Something needs to be addressed about the contractors who repair the pot holes as they literally use breakfast serial which lasts no more than a couple of weeks.
