Surface loose, trip hazard

Reported this some time ago and some repairs were carried out but it is false economy to patch repair. The whole surface of the ramp to the bridge is loose again and is a serious trip hazard. Please give this your serious attention and carry out a proper repair before someone injures themselves and sues the council for damages! The photo only shows a small area of the problem but most of the ramp is in need of re-surfacing.

Reported almost a month ago and still no repairs done or even any signage to warn pedestrians of the hazardous surface. Come on guys get your act together before someone sustains an injury and this time please do a proper job and put full sheets of surfacing material down instead of these individual pieces that keep coming loose.
