Uneven footpath

As a fairly frequent user of the footpath from Barns Lane to the top of the Hill, I have noticed recently that the concrete slabs have become very uneven (due to tree roots?) and poses a trip hazard to the many young school children and the elderly people who regularly use it. This is particularly hazardous now that it gets dark earlier in the day.
Some areas have been patched with tarmac in the past, but it is the uneven cracks between the large slabs that are causing the current problem.
Is it possible for some remedial action to be taken ASAP please
