Sloping pavement surface degraded

The pavement surface (see picture is on a slope and although does not appear too bad in the photo is quite poor - worse when raining. It is used by many young children and parents with buggies due to its proximity to a primary school. My son who has ASD nearly tripped over on it on the way to school.

Thanks for actioning this. Work has been carried out but:
I) the filled in bit is done poorly and lumpily so that it sticks out thus still creating an uneven surface. Can it not be made flush with surrounding pavement when fixed? See photo ii) There are some other bits right by the filled in part which are also potholed. Why were these not done at the same time? Common sense. See photos.
