Potholes round TVWB Access points and linear pothole in road

The above pictures shows the state of the TVWB Access points and a linear pothole in the road. Pic A shows where my disabled wife's walker got caught in the pothole and deflected to the right, as she is blind in one eye she has limited depth of field so didnt notice it as a hazard. This put her off balance, which she was unable to correct due to mobility problems, taking her into the linear pothole ( pic C ) causing her to fall in front of a car. Grid Ref :- 606250 426804. Please take this as Formal Notification of a Highways problem. Pic B shows a second TVWB Access Point in a similar condition. Please note that I have taken a copy of this message. Mr M Williams
