Dangerous crumbling/ cracked footpath

I’ve lived here 4 months but I suspect it’s been like it a lot longer from looking at it. It is a busy path used by parents at the primary school, the path leads to the nursery entrance. I have lost count of the amount of times myself and my 3 year old have tripped on this path, and it is easier for me to push my pram across the grass than the path. It is dangerous. Massive cracks, parts of it have crumbled away, there is a big bump up on the path alongside the road.
