Large manhole cover loose and extremely noisy

A series of circa 3 steel manhole covers are causing excessive noise and disturbance on the main A417 Reading road when vehicles pass over them - at least one is loose and moving in its mount. The noise is extremely loud, especially when heavy goods vehicles pass over the covers, and is an environmental noise nuisance and as the covers are clearly moving they pose a health and safety risk to road users, especially cyclists and motorcyclists.

No action still taken on this despite it originally been reported 4 or 5 months ago in a previous report - unacceptable. The noise is a real nuisance and is as a result of major roadworks that were carried out on the A417 circa 1 year ago not reinstalling the covers correctly. The cover(s) are getting looser and noisier each week - needs investigation and repair please!

Please provide pictures (from a place of safety) and exact location as have attended 3 times now and still cannot find the manhole that is causing the issue. Due to it being a dangerous road, it would be more beneficial to know the location to ensure me being able to deal with it swiftly.

Many thanks for looking into this and actively dealing with it - it was reported many months ago and is somewhat changeable in the degree of noise it emits. Virtually all trucks and some cars (that are positioned near the verge as they travel over it) make a loud metallic banging noise as they drive over, where as cars that are positioned more towards the centre of the road do not create a noise... It also seems as though the noise can be worse sometimes than others, suggesting trucks may be moving the cover around within its frame, hence the variation in the noise emitted.
Its a set of 3 large cast iron covers, on the RHS of the zoomed out picture (between the two trees as the picture is framed) and clearly visible in the zoomed in picture. Would really appreciate you getting to the bottom of the cause of the noise and making some alterations / adjustments to resolve it.
