Pothole around large 3 piece manhole, manhole and frame now loose and noisy

This manhole was installed in the main road A361 opposite Merrivales Lane by BT in early 2017 and was initially problem free. However since the snow in early Dec 2017 the tarmac has come out around the manhole support frame and a pothole has developed. The frame now moves up and down when cars go over it and makes a very loud 'clack clack' noise. The noise is so loud it is clearly audible in the house despite good double glazing and prevents us sleeping, our property is roadside. It is also detrimentally affecting our bed and breakfast business due to the noise.

This has got a lot worse, sharp edges of manhole exposed. Will rip a tyre easily - move to URGENT
